What is this tier?

This tier provides fully-subsidized, rent-free housing in AdvitCare-managed apartment communities. It is designed as a rapid re-housing solution to offer stability. The goal is to provide a safe, secure home base, allowing individuals, couples and families the time and opportunity to build resources, increase income, and eventually transition to other stable, long-term housing. Support services are integrated into this program to help residents achieve their goals.

Who is it for?

This program is specifically for individuals, couples, and families who are currently experiencing homelessness, are at imminent risk of losing their primary nighttime residence or cannot afford their primary residence. This includes:

  1. Individuals/couples/families living in emergency shelters, transitional housing, or places not meant for human habitation (e.g., cars, parks).

  2. Those who are being evicted within 14 days and lack the resources or support networks to secure new housing.

  3. Individuals fleeing domestic violence, sexual assault, or human trafficking who have no other safe housing options.

Qualifications for this tier?

To be eligible, applicants must meet the following criteria:

  1. Housing Status: Must be verified as literally homeless or at imminent risk of homelessness, according to HUD (U.S. Department of Housing and Urban Development) definitions.

  2. Income Level: Total household gross income must not exceed 30% of the Area Median Income (AMI) for your location and family size. (This is considered "Extremely Low-Income").

  3. Citizenship: Applicant must be a legal citizen or legally working/residing or have eligible immigration status in the countries/locations this tier is accessible in. [see below]

  4. Background Check: Applicants must consent to a criminal and mental background check. Certain criminal convictions may impact eligibility if they pose a demonstrable threat to the health and safety of other residents.

  5. Commitment to Partnership: Applicants must agree to partner with an AdvitCare Housing case manager to develop and work on a Housing Stability Plan.

How to Apply?

Gaining access to this tier is done through a "Coordinated Entry" system to ensure fairness.

  1. Initial Contact: The first step is to contact your local hotline or a designated homeless service provider in your area. They will conduct an initial screening to determine if you match the definition of homeless.

  2. Needs Assessment: You will be asked to provide a any type of proof of your situation to verify your eligibility and understand your situation.

  3. Referral to AdvitCare Housing: If you meet the criteria for this tier and there is an opening, you will formally be referred to the AdvitCare Housing intake team.

  4. Intake and Verification: An AdvitCare specialist will contact you to complete the final application and verify all of your documentation.

What You Need

Having any of these documents ready will speed up the process. If you don't have them all, our team can help you obtain them.

  1. Proof of Identity: Valid photo ID (Driver’s License, State ID, Passport) for all household members, birth certificate of the children.

  2. Proof of Identity: Valid ID or birth certificate for emancipated minors.

  3. Proof of Housing Crisis: An eviction notice, a letter from a shelter, or other formal documentation of your homeless status.

  4. Income Verification:

    1. Recent pay stubs (if employed).

    2. Benefit award letters (e.g., Social Security, SSI/SSDI, VA benefits, TANF).

    3. A signed statement if you currently have no income.

  5. Household Information: Names, dates of birth, and relationship to the head of household for everyone who will be living in the apartment.
